3. Phone number whereby we may reach you to confirm your registration
4. Courses for which you are registering. (See flyer for upcoming semester.)
5. Credit or Audit. (Credit means you will meet all requirements of the course and, thus, the course will count toward graduation. Audit means you do not intend to meet all requirements, but just want to "sit in.")
